71C Velvet Drive dislodged front shaft seal

I have a 71C Velvet drive that was rebuilt approximately 10 hrs ago. It start losing fluid very quickly. Upon removal it became evident that the front shaft seal had dislodged. Is there a likely cause for this other than possible faulty installation of the seal? Any assistance is appreciated.
